The drug blocks receptors that are sensitive to a compound released during allergies - histamine. The result of this process is tissue swelling, spasm of smooth muscles, stagnation of blood in small vessels, itching and other allergy symptoms.

Fenistil is able to block the effects of kinins and exhibits minor anticholinergic activity. Clinically, this manifests itself in “drying” of the mucous membranes, which is sometimes used in the treatment of ARVI with increased secretion of the nasal mucosa. The drug has a slight sedative effect and does not have antiemetic activity.

The gel is intended for topical use. It eliminates skin manifestations of allergies without producing systemic side effects such as drowsiness or anticholinergic effects. The antipruritic and mild cooling effect appears within a few minutes after application to the skin.

In low concentrations, it has a stimulating effect on histamine methyltransferase, which leads to the inactivation of histamine. It has a high affinity for H1 receptors and is a mast cell stabilizer. Dimetindene maleate does not affect H2 receptors. It also has local anesthetic properties.

Dimetindene maleate is an antagonist of bradykinin, serotonin and acetylcholine. It exists as a racemic mixture with R - (-) - Dimetindene, which has more pronounced H 1 -antihistamine activity.

Dimetindene maleate significantly reduces capillary hyperpermeability associated with immediate hypersensitivity reactions.

In combination with histamine H2 receptor antagonists, it suppresses almost all types of histamine effects on the circulatory system.

Studies have shown that the effect of a single dose of 4 mg dimethindene drops on skin reactions is determined up to 24 hours after administration of the drug.

Pharmacokinetics. The systemic bioavailability of dimethindene in the form of drops is approximately 70%. The body's initial response to the drug is expected within 30 minutes after administration, the maximum response is expected within 5:00. After taking the drops, the maximum concentration of dimethindene in the blood plasma is reached within 2:00.

At concentrations from 0.09 to 2 μg/ml, the binding of dimethindene to plasma proteins is approximately 90%. Metabolic reactions of dimethindene include hydroxylation and methoxylation.

Its half-life is approximately 6:00. Dimetindene and its metabolites are excreted by the liver and kidneys.

Preclinical studies on safety, toxicity and genotoxicity have not revealed any risk when using the drug in humans. Studies on rats and rabbits did not reveal a teratogenic effect of the drug. Also, studies in rats showed no effect on fertility or fetal and neonatal development when administered at doses 250 times higher than human doses.

Indications

Symptomatic treatment of allergic diseases: urticaria, seasonal (hay fever) and year-round allergic rhinitis, allergies to drugs and food.

Itching of various origins, except those associated with cholestasis. Itching in diseases with skin rashes such as chicken pox. Insect bites.

An auxiliary remedy for eczema and other itchy dermatoses of allergic origin.

Contraindications

Hypersensitivity to dimethindene maleate or any other component of the drug. The patient has duodenal/pyloric stenosis.

Proper safety precautions for use

As with other antihistamines, caution should be exercised when using Fenistil in patients with glaucoma, with urinary disorders, incl. with prostate hypertrophy, as well as with chronic lung diseases.

As with all H1 receptor and partially H2 receptor antagonists, this drug should be used with caution in patients with epilepsy. Antihistamines can cause agitation in children.

Animal studies have not revealed the teratogenic effect of dimethindene and any direct or indirect harmful effects on pregnancy, embryonic development and/or postnatal development. However, since there is currently no information on the use of the drug during pregnancy, Fenistil drops can be prescribed only when absolutely necessary. It is not recommended to take the drug during breastfeeding. If necessary, breastfeeding should be stopped during treatment.

Overdose

In case of an overdose of Fenistil, oral drops, as well as other antihistamines, the following symptoms may occur: depression of the central nervous system (CNS), drowsiness (mainly in adults), stimulation of the central nervous system and anticholinergic effects (especially in children), including agitation, ataxia , tachycardia, hallucinations, convulsions, tremors, urinary retention and fever. Also after this, the development of arterial hypotension, coma and cardiorespiratory collapse is possible.

There is no specific antidote for an overdose of antihistamines. Standard measures should be taken, including the patient taking activated charcoal, a saline laxative, and measures should be taken to maintain the functions of the cardiovascular and respiratory systems. The use of stimulants is not ordered. Vasoconstrictors can be used to treat arterial hypotension.

Side effects

The main side effect of the drug is drowsiness, especially at the beginning of treatment. In very rare cases, allergic reactions may occur.
